Fiscal Intermediaries are insurance companies that the Health Care Financing Administration (HCFA)—now known as the Centers for Medicare and Medicaid Services or CMS—hires to process claims for reimbursement for the rehabilitation services that we provide to patients in health care facilities. Over the last 20 years, I have worked and lectured in the area of Medicare Appeals for Denied Payment for our professional services in Speech-Language Pathology and Audiology, and for Physical and Occupational Therapy services. In the 29 states where I have addressed problems associated with denied payments hardly, if ever, have I seen numerous denied claims from multiple states for the same reason at the same time. What we have seen, and continue to see, are trends across the USA. A trend that I noticed on the West Coast then occurred in other states from the same Intermediary. This information is useful to know, because we then teach our clinicians to document their treatment using the information we learned from the Intermediary’s reason(s) for denying claims.
